Lavender's book is available at BLENKO GLASS in Milton, West Virginia. If you don't see the book ask for it. Lavender is distributed statewide in WV by WV Book Company, and recently got juried into Tamarack and Kentucky Artisan Center, which is like the Tamarack of Kentucky.
